    misfire/hesitation

Occasionally, when driving at low RPMs (between 1000-2000), and I press down on the gas pedal rapidly to about 1/2 or 3/4 throttle, the engine will misfire and hesitate for a moment and I will feel/hear a single "backfire" sound coming from right under the floor. (No check engine light.)

How many miles on your van? Also what are the condition of the spark plugs? I had something similar happen and one of the plugs had a chip broke from it. Once the plug was changed - it resolved the issue.

May be first signs of a weak fuel pump. Backfiring at the intake usually indicates a lean fuel condition. Could also be partially clogged injectors. Anything that will restrict fuel flow.

If it were me, checking the plugs might be one of the first places to start.
